AFTER SCHOOL AGREEMENT AND INFORMATION FORM

ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT (“AGREEMENT”)

Please read, fill out, & submit the form below if you wish to have your child participate in our after school program.

Transportation of a Minor Agreement:

I hereby authorize RAD Gymnastics to pick up my child from his or her school on the pre-selected days I have (or will at a future time) signed up for. I understand that the assumption of risk and indemnity for transportation is consistent and understood to be equal with the “Activity Agreement” found below.

I further understand that in addition to this authorization, I am responsible for informing the appropriate school-officials and arranging the necessary paperwork as is required by my child’s school. I understand that if there is any important information that RAD Gymnastics should know about my child, I will list it below as is necessary.

Please be aware that in the event of vehicle issue and/or emergencies, your child may be picked up in a company-marked staff POV (Personally Owned Vehicle). We do not necessarily inform families each instance that this occurs, but vehicles are always marked and school officials are always aware.

No Pick Up Days:

I understand that it is my responsibility to notify RAD Gymnastics in advance if my child is not in need of pick up on a day she or he is pre-arranged for pick up.

I also understand that RAD Gymnastics follows the Volusia county school operational calendar. All “no-school days” found within that calendar are days that RAD Gymnastics after school program is closed. Most “no-school days” RAD Gymnastics offers a day camp available for an additional but reduced cost. On “no-school days” that RAD Gymnastics is open, After School students are welcomed to come for the normal operating hours of 3-6pm

Important Note About Make Ups: Days & times follow the Volusia County Public School operating calendar. If VCPS is out, the After School program is closed. We do not offer make-up days for the After School Program.

Gymnastics and all of its auxiliary and offspring programs are relatively safe activities, especially when considering the safety measures, professional equipment, and high-level training RAD Gymnastics utilizes and enforces. However, any time an activity involves height and momentum, there is a risk of minor to severe injury
In consideration of participating in RAD Gymnastics, I represent that I understand the nature of this these activities and I hereby for myself, my children, my heirs and executors affirm that if any of the aforementioned should partake in physical activity at RAD Gymnastics, the same shall be qualified, in good health, and in proper physical condition to participate in such activities. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity. I fully understand that this, similar to all other sports and activities, involves risks of bodily injury, including permanent injury, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the “Releasees” named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I incur as a result of my participation in the activity.

I further acknowledge, understand, appreciate and agree that my child’s participation may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my child’s participation and exposure.

I hereby release, discharge, and covenant not to hold responsible RAD Gymnastics, its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the activity takes place, (each considered one of the “RELEASEES” herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the “Releasees” or otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the Releasees from any loss, liability, damage, or cost, which may incur as the result of such claim. Furthermore I hereby authorize RAD Gymnastics to utilize any photography, video, or other likeness of my child for advertisement use, promotional use, training education, and/or any other reasonable purposes that RAD Gymnastics may see fit.

Drop Notice Requirement:

ALL MEMBERS must submit an Online Drop Request via the family portal. Drop requests will not be processed via phone, email, or text.

Refund Policy:

Rad gymnastics has a no refund policy for most of our programs. In most instances, a credit may be placed on the family’s account for future use.

Recreational Classes:

I understand that it is at the full discretion of RAD Gymnastics staff to make recreational class scheduling decisions. I realize that my child will participate in two recreational classes each week but that the schedule and time for the class may change based on After School scheduling needs. If I have special requests relating to recreational classes I understand that I need to speak with management. . Requests cannot always be fulfilled and are based on current availability.

Homework Guidance:

Although we do not tutor After School participants, we do our best to offer good homework guidance. Please select one of the options below in order for us to know how you would like us to proceed with your child.

Absenteeism:
Due to the fact that After School is billed on a weekly basis, there is no change in price if a class is missed. This includes if a child is sick, away for a class field trip/sporting event, or on vacation. Parents MUST Email or Text (386)668-7234 when children are absent from school and do not need to be picked up. We are very cautious about making sure all students are accounted for at the school. If we are not sure, then we must go to the front office to find out about the child. This inevitably makes us late for our next pick up or class. a. First offense: Parent will be called with a friendly reminder b. Second offense, ect.: A $20.00 fee will be applied for the second and every time after.
Attire:
It is the parents and students’ responsibility to make sure the child has the appropriate clothing for the class they will be participating in on each day. Gymnastics: Leotard for girls, shorts for boys. Jeans are never acceptable attire for any classes we teach, including open gym. Long hair must be worn up (this includes boys). NO jewelry is permitted with the exceptions of post earrings (No hoops). Please note that your child will be physical EVERYDAY at RAD, at the minimum; please pack at least gym shorts and shirt. Your child should be bringing clothing in their backpack EVERYDAY.
School Holidays:
During school Holidays, Teacher Duty days, Martin Luther King’s Birthday, Veterans Day and Presidents Day, there will be NO After-School Pick-up. There will, however, be a full day of camp offered. The cost of this camp will be discounted for the Monthly ASP participants. The fee for a day of camp is $50.00 and will be discounted to $25.00. There will be NO camp discount for those who pay weekly tuition. During Multiple days off: Thanksgiving, Christmas Break, Spring Break tuition will be Pro-Rated for the partial week or not charged for full weeks off. We will offer Camps for the entire time out of school. In this situation there will be NO discounts offered for the week of camps.
Inclement Weather:
If a weather event such as a hurricane is to happen or if we do not have power, there will be No After-School Pick-up. In this situation there will be No discount for tuition for that week.
Electronics:
We DO NOT allow ANY type of electronics besides those required for school work. Our goal is to keep your children active and not transferring them from the couch to a gym mat. This will also reduce the arguments about sharing personal games, as well as, losing games and devices. If a device is needed for schoolwork, arrangements can be made.
Below please list any allergies or medical concerns RAD Gymnastics should be aware of. Please be as detailed as possible.
Below please list any special requests and/or notes you would like to convey to the After School staff including the time and class type you would like your child to have. Please note that it is not a guarantee that we will be able to get your child in the requested time and class type, however if we are unable to we will place your child on a waitlist.

I understand that I am signing this document regarding my child who is named below and that all which is written above applies to this agreement with regard to my child and/or other children who I am signing for below.
